Nurse Practitioner | Family Practice Location: Hartford, CT Employer: All Medical Personnel Pay: Competitive weekly pay (inquire for details) Contract Duration: 2 Weeks Start Date: ASAP About the Position Specialty accepted: Nurse Practitioner, Physician Assistant (Adult, Family, and Geriatric accepted) Start date or shifts: ASAP – ongoing Daily schedule: Monday–Sunday, 7:00 AM–7:00 PM | 5–8 in-home assessments per day Practice setting: In-Home Health Assessments Types of procedures: Health and wellness assessments, including H&P evaluations, medication reviews, vitals, lab screenings (finger stick, urine sample), and fecal kit distribution. Number of patients: 5–8 per day Ages: Geriatrics Additional information: Providers must be available at least two full days per week (not necessarily consecutive) and include one weekday; weekend-only availability is not accepted. Providers must hold a license in the state of assignment and one additional state (excluding CA, CO, CT, MA, ME, MN, NH, NY, RI, VT, WA) and reside outside the state of assignment. Candidates from neighboring states are preferred. Flights are provided for those committing to a minimum 10-day assignment. Travel benefits include hotel coverage for travel beyond 75 miles, rental car for 30 weekly assessments, and transportation reimbursement as applicable. Weekly $ incentive available for meeting visit goals.
