Senior Care Nurse Practitioner - Burlingame, CA | Geriatric Primary Care

Opportunity Information

Sutter Senior Care is seeking a full-time Nurse Practitioner to join our geriatrics-focused primary care team serving complex Medicare patients.

Location: Burlingame, CA

Highlights:

• Collaborative, team-based care model working in dyad partnership with a geriatric physician

Responsibilities:

• Provide comprehensive geriatric assessments, diagnoses, and treatment plans

• Manage acute, chronic, and complex medical conditions, including multimorbidity

• Deliver preventive care, screenings, and medication management (including polypharmacy monitoring)

• Perform in-basket management , including patient communications, results review, and follow-up

• Support transitions of care (hospital/SNF follow-up)

• Lead proactive panel management and care gap closure efforts

Requirements

• Minimum 2-3 years of clinical experience preferred; geriatrics or complex care experience strongly preferred

• Current California State NP License

• Current Nurse Practitioner Furnishing Number (for Nurse Practitioners only)

• Nurse Practitioner furnishing number of NP's

• Active DEA registration

• Experience in primary care, geriatrics, or value-based care models

Preferred:

• Experience with high-risk senior populations or panel-based care

• Familiarity with Epic or other EHR systems

• Experience with care management, transitions of care, or home-based care delivery
