Physical Therapist

Licensed Physical Therapist (PT) PositionWe are seeking a licensed Physical Therapist (PT) to provide in-home physical therapy services to patients in the Grand Junction.The Physical Therapist will deliver skilled physical therapy services in patients' homes in accordance with physician orders, payer requirements, agency policies, and applicable state and federal regulations.This role supports patient mobility, safety, and functional independence through individualized, evidence-based care.Clinical ResponsibilitiesPerform assigned physical therapy visits, including evaluations, Start of Care (SOC), reassessments, recertifications, and discharge visitsDevelop and implement individualized physical therapy plans of care based on patient needs and physician ordersProvide skilled interventions such as gait training, balance training, therapeutic exercise, mobility training, and fall preventionEducate patients and caregivers on treatment plans, home exercise programs, and safety strategiesCommunicate patient progress, concerns, or changes in condition to the agency's interdisciplinary care teamCollaborate in the clinical oversight of Physical Therapist Assistants (PTAs), as applicable and permitted under Colorado law and agency policy.
