We are looking for YOU!Job Summary:Nurse Care CoordinatorResponsible for monitoring, assessing, and addressing health and medical needs for Mountain Valley Clients in the Garfield, Pitkin, Eagle, and Lake Counties. The Nurse Care Coordinator supports medical staff in their duties and responsibilities. The Nurse Care Coordinator provides leadership by demonstrating a strong work ethic, solution-oriented thinking, open minded, active listening, good judgement, decisiveness, accountability, and strategic thought.Hours: Full Time, 40 hours per weekWho are we:Our Mission at Mountain Valley Developmental Services is to encourage and support individuals with developmental disabilities, enhance their ability to live, learn, and work in an independent and inclusive manner, and to educate the community about their contributions and capabilities.MVDS supports adults and children in Eagle, Garfield, Lake and Pitkin counties with intellectual and developmental disabilities (I/DD). Our work provides life span services for these individuals, their parents and family members, guardians, colleagues at work, neighbors, and friends to promote independence, quality of life, and rights protection.We take pride in our person-centered care and deep commitment to supporting persons with developmental disabilities. Every day employees strive to help clients live, learn, & work as independently as possible.Responsibilities:Staff Training & Competency – Provide and evaluate training for medical and direct support staff, including QMAP classes, delegation tasks, and health protocol implementation.Health Monitoring & Evaluation – Conduct routine site visits, assess medical needs, review vitals, complete annual case reviews, and provide guidance during emergencies or treatment questions.Direct Medical Care – Perform clinical tasks such as blood draws, ear lavages, and flu shots; develop and manage individualized health and nutrition protocols in collaboration with providers.Healthcare Coordination – Facilitate hospital admissions/discharges, oversee psychiatric services, and monitor individuals on psychotropic medications including tardive dyskinesia screenings.Appointment Oversight – Coordinate and monitor follow-up on medical appointments and treatments, ensure accurate implementation of provider orders and documentation standards.Communication & Advocacy – Act as a liaison between staff, families, medical professionals, and pharmacies to ensure clear, respectful, and timely communication regarding medical care.Documentation & Compliance – Maintain thorough medical records, review communication logs, and ensure compliance with health documentation procedures and standards.Administrative & Regulatory Duties – Update healthcare procedures to align with regulations, manage prescription disposal, administer TB tests, and report medical complaints to leadership.ESSENTIAL FUNCTIONSConduct and record reviews of all residents who have not expressed interest in self-administering their medications to determine them incapable of safely self-medicating. (6 CCR 1011—Chapter 8 14.5 B)Conduct and document an annual capability assessment of residents who are self-medicating. (6 CCR 1011—Chapter 8 14.5 B 2 (c))and more frequently when there are issues with non-compliance, misuse, or inappropriate use of known medications, or when there are changes in resident’s medications, routines, or circumstances that may impact their ability to self-administer. (6 CCR 1011—Chapter 8 14.5 B 2 (a-c))Provide and document training to residents who are self-medicating as needed based on monitoring and assessment results.Review and document findings of the medications & dosages of self-administering residents on at least a quarterly basis. (6 CCR 1011—Chapter 8 14.1)Train, determine the competence of, and monitor QMAPs who provide specialized care in licensed settings. (6 CCR 1011—Chapter 8 16.2)Ensure each resident who utilizes gastrostomy services has a comprehensive, written protocol in place. (6 CCR 1011—Chapter 8 17.2)Provide training of QMAPs on gastrostomy services initially and upon changes made to it & document accordingly. (6 CCR 1011—Chapter 8 17.3- 17.5, 17.7)After initial training QMAPs on gastrostomy services, conduct observations of QMAPs providing gastrostomy services quarterly for 12 months and then every 6 months after the first year. Document accordingly. (6 CCR 1011—Chapter 8 17.3- 17.6)Monitor for individuals taking psychotropic medications for tardive dyskinesia and other abnormal movements and effects, neurotoxicity, and neuroleptic malignant syndrome as time allows or as requested. Document accordingly. (10 CCR 2505-10 8.609.6. D. 8 e.)Teach QMAP classes and perform observations of staff; as time allows or as requested. (6 CCR 1011-1 Chapter 24)Perform clinics including but not limited to nail and ear irrigation as time allows or as requested.Participate in activities that contribute to ongoing development of the profession’s body of knowledge.Visit people on site routinely and as needed to assess and determine medical needs and health.Perform Ear lavages, blood draws, flu shots and other nursing tasks as needed.Assist in the development and maintenance of individualized health care, therapeutic and nutritional protocols in collaboration with health care providers and MVDS team.Coordinate and facilitate psychiatric services, including follow-up of medical recommendations, order changes, updating informed consents and obtaining dictation.Monitor, create and respond to TLogs and GERs with relevant medical information.Obtain written orders from physicians to ensure they meet required standards. Obtain clarification on orders as needed and act as a resource to staff and families in carrying out orders.Serve as a liaison between medical professionals, pharmacies, alternative decision makers, families and staff to communicate needs related to health/medical issues of people supported as needed.Perform ongoing audits for persons supported and notify appropriate staff and/or physicians of any concerns.Occasionally attend staff meetings at group homes or day program sites to discuss the healthcare needs of the individuals supported at that site with the direct support staff.Provide medical advice to staff in the event of a medical/medication questions, treatment question, medical emergency or development of a serious new health problem to determine treatment outside, during and after-hours as needed.Review and clarify doctor’s orders as needed.Exercise informed judgment and use individual competence and qualifications in performing duties.Communicate in a direct, clear, and informed manner.Administer pre-employment TB tests as needed.OTHER REQUIRED DUTIESStandards of Behavior/ConductRespond to all requests for information or communication in a timely manner.Follow all policies within the MVDS Employee Handbook.Report all on-the-job injuries to immediate supervisor and Director of Human Resources within 24 hours of the occurrence and provide a detailed account of the events leading to the injury.Respond as a Mandatory Reporter. Report all incidents of suspected client rights violations, mistreatment, abuse, neglect or exploitation immediately according to Colorado state law and MVDS policies and procedures.Attendance and SchedulingReport for work on-time and as scheduled.Enter daily, accurate timekeeping records in timekeeping database.Attend all regular staff meetings and other meetings as requested by supervisory or administrative staff members.Work schedule as assigned.Have all time-off requests pre-approved.Have all over-time hours pre-approved or demonstrate they were due to an emergency.Work at any MVDS facility and be responsible for own transportation to work.Complete all mandatory new hire requirements and training as identified by the State of Colorado and Mountain Valley’s training requirements within 90 days of hire and maintain courses and certifications as required.Maintain skills with computers, take advantage of new technology, learn new tools quickly, use technology to enhance job performance and increase productivity. Maintain current understanding of technical processes/equipment.Adherence to policies and proceduresAdhere to MVDS policies and procedures outlined in the policy manual, Employee Handbook, and other employee policies.Complete other duties and responsibilities as assigned to ensure effective functioning of Mountain Valley’s programming.NOTE: This position description is intended to indicate the basic nature of the position and to provide examples of typical duties that may be assigned. The position requires occasional lifting, carrying, pushing or pulling objects up to 25 lbs. The position also requires ordinary talking and hearing capability; full vision capabilities; and the ability to read/comprehend, write, communicate orally, and reason and analyze on a constant basis, and to perform calculations frequently.
