WCS Healthcare is actively seeking an experienced Registered Nurse (RN) with UAS Assessment experience to join a growing Home Health team serving Suffolk County, Long Island .

What You'll DoAs a UAS Assessment Nurse , you will help patients access the home care services and support they need to remain safe and independent in their homes.
Responsibilities:- Conduct UAS-NY assessments in patients' homes throughout Suffolk County
- Evaluate physical, cognitive, psychosocial, and functional needs
- Complete accurate and timely assessment documentation
- Determine eligibility for home care and long-term care services
- Develop recommendations based on assessment findings
- Collaborate with physicians, care managers, caregivers, and interdisciplinary teams
- Educate patients and families on available services and community resources
- Maintain compliance with agency policies and New York State regulations
Active New York State Registered Nurse (RN) License
Current UAS-NY Certification
Previous UAS Assessment experience
Strong clinical assessment and documentation skills
Excellent communication and time management abilities
Preferred:- Home Care experience
- MLTC experience
- Case Management experience
- Community Health experience
- Full-Time
- Monday-Friday
- 9:00 AM-5:00 PM
- 3 days conducting in-home assessments throughout Suffolk County
- 2 days working remotely from home
