Master Social Worker - MSW

Purpose and Scope Provides psychosocial services to patients treated by the facility, including both in‑center and home dialysis patients, utilizing Social Work theory of human behavior and accepted methods of social work practice. Works with the health‑care team to promote positive adjustment, rehabilitation, and improved quality of life for patients and their families. Supports the Fresenius Kidney Care (FKC) commitment to quality indicators, outcomes, and quality assessment and improvement activities related to patient satisfaction, quality of life, and process improvement. Principal Duties & Responsibilities Assess patients’ psychosocial status, strengths, and areas of need that may affect rehabilitation and optimal treatment outcomes. Participate in care planning with patients and the health‑care team to identify effective interventions that support rehabilitation, treatment goals, and improved quality of life. Utilize FKC patient education programs, established social work theory and methods, social work‑focused interventions, and quality of life measurement instruments to address barriers and meet patient treatment goals. Provide monitoring and interventions to help patients adjust to dialysis and achieve optimal psychosocial status. Offer supportive counseling services within the scope of clinical training and state license. Provide educational counseling to patients seeking transplantation and assist with referral to community resources (home health services, vocational rehabilitation, etc.) to facilitate optimal outcomes. Maintain current knowledge of local vocational/educational rehabilitation programs and assist patients with referrals to remain employed, become employed, or receive education. Assess patient awareness of advance directives and facilitate discussion of advance directive wishes with the health‑care team and family/support persons. Assist in discussions of patient DNR status to ensure informed decision‑making. Document all interactions and interventions per MSW policies and procedures. Quality Provide psychosocial support and social work‑focused interventions to address non‑adherence, quality outcomes, and quality of life concerns based on acuity level. Participate in monthly quality review meetings with the interdisciplinary team. Report on quality indicators related to adherence, including missed and shortened treatments, quality of life trends, and service recovery. Patient Education Assess patient knowledge of kidney disease and identify barriers that may affect adherence. Collaborate with patients, families, and the health‑care team to provide education tailored to learning styles and needs. Provide information about all treatment modalities and facilitate the transplant referral process. Offer ongoing education regarding psychosocial issues related to ESRD and available support services. Review patient rights, responsibilities, and grievance procedures to ensure understanding. Patient Admission & Continuity of Care Review patient rights and responsibilities, grievance procedures, and important contact information during admission. Support clinic scheduling to maximize adherence and adjustment. Interview patients to identify root causes of discharge requests and communicate findings to operational leadership. Insurance & Financial Assistance Act as liaison with insurance coordinators to address insurance issues related to dialysis, transplantation, and other coverage changes. Educate patients on payment options, including federal, state, commercial insurance, state renal programs, AKF HIPP, and entitlement programs. Refer patients to the patient billing solutions department for billing questions or concerns. Staff Related Assist with interview processes and hiring decisions when requested by the Social Work Manager/Senior Manager. Maintain updated patient resource lists and provide staff training on psychosocial topics. Participate in weekly team huddles and address urgent patient issues with staff. Adhere to caseload guidelines based on state regulatory requirements. Perform other related duties as assigned. Physical Demands & Working Conditions Travel may be required for multiple facilities or home visits. Ability to perform essential functions with reasonable accommodations for disabilities. Education & Required Credentials Master’s degree in Social Work. Valid state license meeting scope of practice and licensure requirements. Experience & Skills 0–2 years of related experience.
