Registered Nurse (RN) VA Community Care (VACC) The Veterans Health Administration was seeking a Registered Nurse for VA Community Care at the Oscar G. Johnson VA Medical Center in Iron Mountain, MI. This position supports high-quality and timely care for Veterans receiving services in the community by coordinating consults, triaging requests, reviewing medical documentation, guiding Veterans through care options, and serving as a liaison between VA and community providers. Location: Iron Mountain, MI Vacancies: 1 Salary: $65,344 - $122,879 per year Pay Scale & Grade: VN-00 Promotion Potential: None Work Schedule: Full-time, Monday - Friday, 8:00am - 4:30pm Appointment Type: Permanent Telework: Not available Remote/Virtual: Not available Travel Required: 25% or less Relocation Expenses: Not reimbursed PCS: Not authorized Drug Test: Required Security Clearance: Other Position Sensitivity/Risk: Non-sensitive / Low Risk Union Representation: Yes Key Responsibilities: Provide and/or supervise direct patient care and remain accountable for all elements of the nursing process. Assess, plan, implement, and evaluate the effectiveness of Veteran care. Support high-quality and timely care for Veterans receiving services in the community. Coordinate care focused on patient education, self-management, and customer satisfaction across the continuum of care. Collaborate with interdisciplinary team members to influence care outcomes. Participate in unit and program-level quality improvement processes, initiatives, and customer service programs. Execute a streamlined approach to receiving, triaging, and directing timely coordinated care. Use proficient nursing practice, decision-making, deliberate planning, and critical thinking skills. Apply knowledge of internal referral care processes, clinical review criteria, utilization management standards, clinical documentation requirements, community care regulations, and VHA standards. Assess, organize, facilitate, and guide Veterans through care options including internal VA care and care in the community. Serve as a liaison to internal and community providers. Manage Veteran care throughout the consult process. Support timely access to care and help ensure Veterans who want community care are referred and scheduled appropriately. Work collaboratively within the Office of Integrated Veteran Care to triage Requests for Services. Develop care coordination plans, review community provider medical documentation, communicate care coordination needs, and support Veterans through completion of the episode of care. Use VA-mandated processes and health information systems to assess, document, exchange, and track care coordination information. Support professional development of health care professionals and contribute a nursing perspective in interdisciplinary team discussions. Share knowledge, support colleagues, question clinical practices, and participate in evidence-based practice activities. Basic Qualifications: Must be a U.S. Citizen. Non-citizens may only be appointed when qualified citizens cannot be recruited in accordance with VA policy. Must be proficient in basic written and spoken English. Must be subject to a background/security investigation. Must pass a pre-employment physical examination. Must pass pre-employment drug testing and remain subject to drug testing requirements for this testing designated position. Selective Service registration is required for males born after December 31, 1959. Must complete all application requirements and online onboarding requirements. Must provide acceptable identification for employment eligibility verification. Must participate in the seasonal influenza vaccination program as required for Department of Veterans Affairs health care personnel. Selected applicant may be required to serve a probationary period. Education and Licensure Requirements: Must be a graduate of a school of professional nursing approved by the appropriate accrediting agency. The nursing program must have been accredited by the Accreditation Commission for Education in Nursing or the Commission on Collegiate Nursing Education at the time the program was completed. Foreign nursing education above the associate degree or entry-level nursing education requires formal degree equivalency validation from an accepted evaluation agency. Must have a current, full, active, and unrestricted registration as a graduate professional nurse in a U.S. State, Territory, Commonwealth, or the District of Columbia. Graduate Nurse Technician candidates may be temporarily appointed if they otherwise meet education requirements and are actively pursuing licensure, subject to VA policy. Nurse Grade Determination: Nurse I Level I: ADN or diploma in nursing with no additional professional nursing experience required. Nurse I Level II: ADN or diploma in nursing plus one year of specialized nursing experience equivalent to Nurse I Level I, or BSN with no additional professional nursing experience required. Nurse I Level III: ADN or diploma in nursing plus two years of professional nursing experience with one year equivalent to Nurse I Level II; or BSN plus one year of professional nursing experience equivalent to Nurse I Level II; or MSN with no additional professional nursing experience; or related master's degree with BSN and no additional professional nursing experience. Nurse II: BSN plus two years of professional nursing experience equivalent to Nurse I Level III; or MSN plus one year of specialized nursing experience equivalent to Nurse I Level III; or related master's degree with BSN plus one year of specialized experience; or doctoral degree in nursing or related field with BSN and no additional professional nursing experience. Nurse III: MSN plus two years of specialized nursing experience including one year equivalent to Nurse II; or related master's degree with BSN plus two years of specialized nursing experience including one year equivalent to Nurse II; or doctoral degree in nursing plus one year of specialized experience equivalent to Nurse II; or related doctoral degree with BSN plus one year of specialized experience equivalent to Nurse II. Grade and salary are determined by education, scope, length, and quality of nursing practice experience. Preferred Experience: One year of care coordination, case management, or primary care experience. Physical Requirements: Requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. May require long periods of walking, standing, stooping, sitting, bending, pulling, and pushing. Transferring patients and objects may be required, with mechanical inpatient lifts provided. May involve exposure to infected patients and contaminated materials. May require protective clothing in isolation situations, operative or invasive procedures, or home settings. May occasionally be exposed to patients who are combative due to delirium, dementia, or psychiatric disorders. Must be mature, flexible, sensible, able to work effectively in stressful situations, and able to shift priorities based on patient needs. Must complete annual employee health requirements, such as annual TB screening or testing, as a condition of employment.
