Who we are: Titans Home Healthcare LLC is dedicated to providing compassionate, person‐centered support for individuals in group home settings. We oversee and manage high‐quality residential services, ensuring that every home operates smoothly, safely, and in full compliance with DDD and agency standards. Our team focuses on empowering staff, maintaining exceptional care environments, and promoting the well‐being, dignity, and independence of the individuals we serve.

 

Residential Support

• Maintain a clean, safe, and home‐like environment in alignment with licensing requirements.

• Assist members with personal care needs, including hygiene, dressing, and toileting.

• Prepare and serve nutritious meals based on individual dietary needs and posted menus.

• Perform routine housekeeping tasks such as laundry, sweeping, and sanitizing bathrooms.

• Provide medication reminders and support with health‐related appointments under supervision.

• Follow agency protocols for fire drills, emergency procedures, and infection control.

• Complete accurate daily documentation, shift notes, and communication logs.

Habilitation (HAB) Support

• Implement individualized Support Plans (ISPs) and behavior strategies tailored to each member.

• Assist members in developing Activities of Daily Living (ADLs) and social or recreational skills.

• Promote community integration through outings, activities, and skill‐building opportunities.

• Use teachable moments to reinforce independence, communication, and coping skills.

• Participate in ISP meetings, staff trainings, and ongoing team communication.

• Monitor, document, and report progress, incidents, and any changes in behavior.
