Field Care Manager, Behavioral Health

Humana is seeking a Field Care Manager, Behavioral Health to provide integrated, person-centered support to members in their homes and communities. In this role, you will assess behavioral health and social needs, develop individualized care plans, coordinate services, and collaborate with medical, behavioral, and community providers. You will monitor progress, support medication and treatment adherence, and intervene during crises. Working closely with an interdisciplinary team, you’ll use data and clinical judgment to close care gaps, reduce hospitalizations, and improve outcomes while documenting accurately and meeting quality and compliance standards.
