VN Hospice Community Outreach Liaison Marin County Territory, CaliforniaMonday – Friday | 8:00 AM – 4:30 PM Compensation: $40 – $45 per Hour Position Overview A growing, mission-driven hospice organization is seeking a compassionate and relationship-focused LVN Hospice Community Outreach Liaison to join its expanding Marin County team. This is an excellent opportunity for a hospice clinician who enjoys community engagement, patient advocacy, and relationship-building while continuing to make a meaningful impact on patients and families during critical moments of care.In this role, you will serve as a trusted resource for referral partners, patients, families, and internal care teams by helping educate the community about hospice services and supporting the referral and admissions process. The ideal candidate will bring prior hospice experience, strong interpersonal skills, and a passion for compassionate end-of-life care. Key Responsibilities Support hospice clinical and outreach teams with patient referrals, admissions coordination, and community engagement initiativesBuild and maintain strong professional relationships with physicians, hospitals, skilled nursing facilities, assisted living communities, case managers, and other referral partnersEducate patients, families, and healthcare professionals regarding hospice philosophy, eligibility, and available servicesConduct introductory meetings with patients and families and assist with hospice consent discussionsParticipate in community outreach events, networking opportunities, and educational initiatives throughout the territoryMaintain accurate CRM documentation, referral tracking, and activity reportingIdentify referral opportunities and communicate market feedback and growth opportunities to leadershipServe as a knowledgeable hospice resource for referral partners and internal team membersCollaborate closely with clinical and operational leadership to support exceptional patient experiences and continued organizational growth Qualifications Required Current unrestricted LVN or LPN licenseMinimum of 1 year of hospice experience requiredReliable transportation, valid driver’s license, and current auto insurancePreferred 2–3 years of hospice experience preferredPrior community outreach, healthcare liaison, or customer service experience strongly preferredDesired Skills Desired Skills Strong communication and relationship-building abilitiesComfortable working in both clinical and community-based settingsSelf-motivated with excellent organizational and follow-up skillsPassion for patient advocacy and compassionate hospice care Compensation & Benefits Comprehensive Medical, Dental & Vision Insurance Generous PTO & Paid Holidays 401(k) with Company Match Tuition Reimbursement & Certification Assistance Cell Phone & Mileage Reimbursement Wellness & Gym Membership Reimbursement Career Growth & Advancement Opportunities Structured Clinical Onboarding & Mentorship Program This is an outstanding opportunity for a hospice professional who enjoys building relationships, supporting patients and families, and serving as a compassionate community resource within a mission-driven organization dedicated to exceptional end-of-life care. #J-18808-Ljbffr i4 Search Group
