Days Description NOTE THAT THIS IS A GRANT POSITION FUNDED THROUGH DECEMBER 2026 SSHCO is seeking an experienced and dynamic Project Manager to lead the clinical quality intervention component of the Live Healthy Chicago program. This role will focus on coordinating and managing quality improvement efforts within partner primary care clinics to enhance hypertension control and patient outcomes. The Project Manager will serve as a key liaison between clinical teams, community partners, and program leadership to ensure successful implementation and sustainability of intervention activities. Key Responsibilities:
- Lead planning, coordination, and execution of clinical quality improvement interventions in partner clinics.
- Develop and manage project timelines, deliverables, and budgets in alignment with grant requirements.
- Facilitate communication and collaboration among clinical staff, community partners, and stakeholders to support intervention goals.
- Monitor project progress, identify challenges, and implement solutions to maintain momentum and achieve targets.
- Oversee data collection, quality assurance, and reporting related to clinical intervention outcomes.
- Support training and capacity-building activities for clinic teams to enhance quality improvement skills and practices.
- Ensure compliance with regulatory, ethical, and grant-related standards.
- Prepare regular updates, reports, and presentations for leadership, partners, and funders.
- Contribute to sustainability planning to embed successful interventions into routine clinical practice.
- Bachelor's degree in Public Health, Healthcare Administration, Nursing, or related field required; Master's degree preferred.
- Minimum 3-5 years of project management experience in healthcare, public health, or community-based settings.
