The Home-Based Services Coordinator supervises our case managers and reports directly to the Director, managing program operations in their absence. In this role, you will collaborate with fellow coordinators across Clearbrook programs and maintain vital contacts with families, guardians, and external service providers. You must ensure strict compliance with Clearbrook’s confidentiality policy, facilitate staff meetings for timely communication, and effectively implement the PHI coaching approach with your team. Qualifications The Coordinator must have: Bachelor's degree in a social service related field At least 3 year of experience working directly with individuals with intellectual disabilities Valid IL driver’s license Minimum of 2 years supervisory experience is required At least 2 years of case management experience Full Time Benefits Package Group term life insurance and long-term disability insurance (fully paid for by Clearbrook!)
