Emphasize health promotion and disease prevention using evidence-based practice. Develop and implement individualized plans to support optimal population health and disease management. Demonstrate leadership and collaborate with the Primary Care Team and interdisciplinary team members. Order and interpret labs, tests, and diagnostic studies. Prescribe pharmacologic and non-pharmacologic interventions. Initiate referrals and consults to specialty services. Provide education to patients and their families. Education and Licensure Requirements: Must have a post-masters certificate, masters degree, or doctoral degree as a Nurse Practitioner. Must be a graduate of a school of professional nursing approved by the appropriate accrediting agency. The nursing program must have been accredited by the Accreditation Commission for Education in Nursing or the Commission on Collegiate Nursing Education at the time the program was completed. Foreign nurse practitioner education must be validated through an agency-recognized degree equivalency evaluation agency. Must possess and maintain an active, current, full, and unrestricted Advanced Practice Nurse license to practice as a Nurse Practitioner in a U.S. State, Territory, Commonwealth, or the District of Columbia. Must maintain full and current basic board certification in the specialty area for which academically prepared. Qualifying certifications include Family Nurse Practitioner, Adult Nurse Practitioner, Adult-Gerontology Primary Care Nurse Practitioner, or Gerontology Nurse Practitioner. Certification must be from the American Nurses Credentialing Center, American Academy of Nurse Practitioners Certification Board, or another nationally recognized certifying body. Nurse Practitioner II Qualification Requirements: Must meet the basic education requirement for Advanced Practice Nurse / Nurse Practitioner. No additional experience is required for Nurse Practitioner II, which is considered the entry grade level for Advanced Practice Nurse / Nurse Practitioner positions. Must demonstrate leadership in delivering and improving holistic care through collaborative strategies. Must deliver evidence-based health care and demonstrate proficiency in managing a patient panel, population, or program. Must use research and evidence-based practice to evaluate patient care delivery and contribute to improvements in quality and timeliness of care. Must work effectively as a leader within a team or unit and seek interdisciplinary collaboration as appropriate. Must demonstrate effective communication, professional behavior, cooperation, and teamwork. Nurse Practitioner III Qualification Requirements: Must meet the basic education requirement for Advanced Practice Nurse / Nurse Practitioner. Must have one year of specialized Advanced Practice Nurse / Nurse Practitioner experience equivalent to the Nurse II grade level. Must execute responsibilities demonstrating leadership, experience, and creative approaches to management of complex patient care. Must influence and model delivery of evidence-based care and demonstrate expertise in managing a patient panel, population, or program. Must lead and implement changes that improve quality and timeliness of Veteran care. Must initiate and lead interdisciplinary teams using structured approaches and processes to improve outcomes. Must acquire and disseminate knowledge and skills within or outside the medical facility. Preferred Experience: Two years of Primary Care Provider experience. Physical Requirements: Must be able to perform all activities of the position without restriction. Requires considerable standing, walking, stooping, bending, reaching, lifting, and turning. Requires repositioning patients and assisting with transfers to wheelchairs or stretchers. Mechanical devices must be used when available for heavy or difficult-to-lift patients. May involve pushing or lifting weight in excess of 50 pounds and going up and down flights of stairs. Requires visual and auditory acuity sufficient to provide safe and effective nursing care. May require working with arms above shoulder level or with the neck fully flexed or extended. Must be able to physically control or protect self from physically abusive patients.
