Develop, implement, and adjust individualized patient care plans to ensure clinical goals are met. Serve as the primary liaison across healthcare disciplines to ensure seamless, well‑coordinated patient treatment. Assess and manage discharge planning needs in collaboration with the healthcare team. Qualifications Licensure / Certification / Registration: BLS Provider obtained within 1 month of hire date or job transfer date; American Heart Association or American Red Cross accepted. Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date required. Licensure from the Wisconsin Board of Nursing or current home state license if considered multi‑state/Compact State. Education: Diploma from an accredited school/college of nursing or required professional licensure at time of hire. Preferred qualifications Bachelor of Science in Nursing preferred. Recent Med‑Surg or Endoscopy experience preferred. Employment details Hours: 7:00 AM – 5:30 PM. Expected to float between CSM Hospital GI Departments.
