Visit hospitalized hospice patients to support continuity of care and communication among patients, families, hospital staff, and the hospice team. Respond promptly to referrals, questions, and requests, and follow through until needs are addressed. Provide responsive, professional service to patients, families, hospital partners, and colleagues. Demonstrate sound clinical judgment, strong problem-solving skills, and the ability to manage competing priorities. Uphold AMOREM’s mission, vision, and values through daily interactions and job performance. Participate in Quality Assurance and Performance Improvement activities. Perform other related duties as assigned. Qualifications Bachelor’s degree in nursing. Minimum of two years of nursing experience; hospice experience strongly preferred. Strong clinical assessment, communication, and organizational skills. Ability to build collaborative relationships with healthcare professionals and communicate sensitively with patients and families. Ability to work independently, anticipate needs, and coordinate effectively across teams. Commitment to compassionate care, professional integrity, and expanding access to hospice services.
