Case Manager – Behavioral Health Quadrant Health Group, Weatherford, Texas, United States About this position Provide comprehensive case management services to individuals with mental health and/or substance use disorders. Report directly to the Clinical Director and support residents throughout the treatment process from admission to discharge. Compensation $19–$22 per hour, full‑time. What You’ll Do Assist and support residents through the entire treatment experience, monitor treatment progress, provide crisis intervention, and contribute to evolving treatment plans. Major Tasks, Duties and Responsibilities Conduct comprehensive assessments to identify client needs and develop individualized care plans. Coordinate and monitor the delivery of behavioral health services, including therapy, medication management, and support groups. Connect clients with community resources, such as housing, food assistance, employment services, and transportation. Develop comprehensive treatment plans with client input and participate in treatment planning meetings. Advocate for clients’ rights and needs with healthcare providers, social service agencies, and other organizations. Provide crisis intervention and support as needed. Maintain accurate and timely documentation of client interactions and progress. Collaborate with interdisciplinary team members, including therapists, psychiatrists, and other healthcare professionals. Contact residents’ family members to set appointments or educate them on residents’ status and progress. Monitor client progress and adjust care plans as needed. Participate in team meetings and training sessions. Maintain compliance with all applicable regulations and ethical standards. What You’ll Bring Case Manager Skills, Knowledge and Competencies: Knowledge of mental health and substance use disorders and treatment modalities. Strong understanding of community resources and social service systems. Demonstrated proficiency with computer technology and effective written and verbal communication. Working knowledge of state and accreditation requirements: DHCS – Substance Use Disorders, JCAHO, and regulatory agencies. Understanding of clinical protocol of mind, body, spirit. Familiarity with the philosophy and approach of the 12‑step model of recovery (AA, CA, NA, Alanon). Understanding of professional ethics, boundaries, and patient confidentiality. Basic knowledge of referrals, both in and out of the organization. Ability to communicate clearly with clients, staff, peers, supervisors and resources. Strong clinical and assessment skills. Excellent organizational and time‑management skills. Qualifications Bachelor’s degree in social work, psychology, counseling, or a related field (Master’s preferred). Minimum of 3 years of experience in behavioral health case management. Current and valid Texas license. Experience working with diverse populations. Proficiency in electronic health records (EHR) systems. Current CPR and First Aid certification. Successful completion of pre‑employment requirements, including a criminal background clearance, drug testing, and health screening.
