Job Title: RN Home Health Profession: RN Home Health Specialty: RN Home Health Duration: 13 weeks Shift: Monday to Friday Hours per Shift: 8:00 AM - 4:30 PM Experience: Minimum years of experience required License: Active RN License Certifications: BLS Must-Have: - RN license - BLS certification Description: Under the direction of the Clinical Team Manager, responsible for providing skilled care and case management to patients and families. The RN case manager ensures services comply with the State Nurse Practice Act and professional standards of practice. Utilizes nursing theory and processes in the delivery of care. Communicates and documents observations and assessments effectively. Promotes and maintains the health of individuals through direct care, teaching, counseling, and appropriate rehabilitative measures. Participates in weekend rotation with a requirement of one out of four weekends.
