As a Nurse Practitioner at Carease Health - WI, you will be part of a hybrid role that combines on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation.Responsibilities:Provide high-quality preventative care to residents in skilled nursing facilitiesConduct on-site visits and remote work from home, supported by a dedicated scribe teamBuild strong relationships with facility staff and maintain a patient-centered approachCollaborate with other healthcare professionals to ensure seamless patient careMaintain accurate and timely documentation of patient visits and treatment plansRequirements:Active NP or PA license (in the state of practice)1+ year of experience in Geriatric care and documentation reviewExcellent communication, collaboration, and teamwork skillsMasters-level education in Nursing or a related fieldAbout the Role:Carease Health - WI is a company that truly cares about its employees.
