divh2Certified Home Health Aide With Hospice Experience/h2pWe are currently seeking a Certified Home Health Aide with Hospice experience to join our San Diego team!/ppstrongResponsibilities:/strong/pulliCompletes charting and completion of appropriate documentation timely./liliAssists patients with bath, mouth care, skin, hair and all aspects of personal hygiene./liliAssists patients with transferring and ambulation as needed./liliAssists with prescribed exercises which patients and CHHA have been educated in by appropriate health professionals./liliPrepares meals and assists patients with eating as required./liliAssists patients in toileting including use of bedpans, urinal and commodes./liliPrepares household services that will facilitate the patients self-care at home and are necessary to prevent or postpone institutionalization./liliReports changes in patient condition and needs to the Case Manager or Team Manager./li/ulpstrongQualifications:/strong/pulliHigh School diploma or equivalent. Completion of CHHA program./lili1 to 2 years direct patient care experience in an institution or home care setting./liliAppropriate understanding of patient care requirements within certification./liliBLS required.
