Position: Medical Social Worker As a Medical Social Worker, you will provide psychosocial assessment, counseling, care planning, advocacy, resource coordination, caregiver support, and end-of-life support to hospice patients and their families. What You’ll Do Assess the psychosocial, emotional, financial, and practical needs of patients and families. Develop individualized plans of care based on patient goals and preferences. Provide counseling, education, emotional support, and crisis intervention as appropriate. Connect patients and families with financial assistance, community resources, agencies, and support programs. Assist patients and families with advance directives and end-of-life planning. Provide caregiver support related to stress, anticipatory grief, and bereavement. Advocate for patient rights, preferences, dignity, comfort, and quality of life. Participate as an active member of the interdisciplinary hospice team. Collaborate with physicians, nurses, chaplains, hospice aides, volunteers, and other care professionals. Complete accurate and timely documentation in the electronic medical record. Provide compassionate, patient-centered care in homes, nursing facilities, and assisted living communities.
