Lead Care Manager - LCSW (Licensed Clinical Social Worker)

Job TitleJob position is available for Full Time, 12:00 PM to 08:30 PMYearly Base Salary Pay Range: USD $94972 to $150321$10k Hiring Bonus for eligible external candidates who meet all conditions for payment - this is in addition to the fantastic benefits and compensation package offered by the hospital that begin on your first day of employment.Required QualificationsMaster's Degree in Social Work from an accredited program.Upon hire: California Clinical Social Worker License.2 years of experience in healthcare related field (Acute, Ambulatory, or Post-Acute).2 years of experience in Acute Care Management (Care Coordination) or successful completion of the Transitions in Practice (TIP) program for Care Managers.
