Case Manager

Position will provide services in Macon County, TN. Responsibilities Provide services to enable individuals and families to access medical, social, educational, and other support services, as well as counseling and therapeutic services. The function is assessing, advocating, linking, and coordinating community-based services. Support client educational, legal, vocational, extracurricular, and health goals, promoting achievement of these goals. Conduct regular home or residence visits and meet with client and family members at schools, daycares, job sites, etc. Schedule non‑exceeding 40 hours per week. Qualifications Bachelor’s degree (B.A. or B.S.) in social sciences with 1 year relevant professional experience working with families and children. Bachelor’s degree in another field with 3 years of relevant professional experience working with families and children. Training in crisis prevention initiatives and universal precautions required. Proof of automobile insurance, reliable transportation, and valid driver’s license.
