Sabathani Community Center (SCC) is one of Minnesota’s oldest African American-founded nonprofits & serves a multi-cultural community of more than 50,000 community members in South Minneapolis. SCC was founded in 1966 by a handful of concerned community members seeking to address social and economic disparities in underserved communities in the areas of youth programming, housing, health and education. Learn more at About the role Sabathani Community Center's Health Equity Department works to reduce health disparities and improve access to care for community members through direct service, care coordination, and community education. The department's programming is designed to meet residents where they are, with a focus on culturally responsive, trauma-informed care. Sabathani Community Center is seeking a Senior Outreach Coordinator to lead a Hennepin County-funded outreach program serving African American and African seniors (55+) in South Minneapolis, including seniors with chronic conditions who are not eligible for waivered services, along with their family caregivers and informal support networks. The Senior Outreach Coordinator conducts intakes and individual service plans, coordinates a volunteer Companion Program, facilitates a monthly Caregiver Circle, provides short-term case management and referrals, and manages program data and quarterly reporting. This role exists to honor elders, reduce isolation, and help seniors age with dignity, joy, and independence, and is well suited to someone who builds trust through consistent follow-through and sees the seniors they serve as community members with wisdom, stories, and gifts to share. The physical duties associated with this position are consistent with duties associated with work performed in an office and community setting. What you'll do Intake & Assessment Conduct comprehensive intakes with seniors to identify needs, goals, and barriers, engaging 200 unduplicated seniors annually. Complete individual service plans within 14 days of intake. Schedule quarterly check-ins to track progress and update service plans as needs change. Companion Program Coordination Launch and maintain a Companion Program serving 60 seniors annually. Recruit, train, and support 10 volunteers or peer companions annually, including ongoing recognition and appreciation. Match companions with seniors based on interests, personalities, and schedules. Conduct monthly check-ins with companions, quarterly check-ins with seniors, and bi-annual isolation surveys. Caregiver Circle Facilitation Host a monthly Caregiver Circle using a structured 90-minute format (welcome, guest speaker, Q&A, peer discussion, closing). Book speakers on topics such as dementia care, boundaries, self-care, Medicare navigation, legal planning, and respite options, covering 8+ unique caregiving topics annually. Promote sessions three weeks in advance through flyers, email, social media, partners, and community media, and collect participant feedback after each session. Case Management & Referrals Provide short-term navigation, advocacy, and coordination related to housing, transportation, food, and healthcare. Assist seniors with benefit applications (Medical Assistance, SNAP) and follow up to ensure referrals are completed. Coordinate at least one annual resource fair engaging 50+ seniors and caregivers. Data Tracking & Reporting Maintain demographic, service, and outcome data on all participants in the program tracking system (Salesforce). Submit 100% of quarterly reports to Hennepin County on time, combining data with participant stories to communicate program impact. Conduct progress assessments at intake and every three months, administer annual satisfaction surveys, and co-facilitate two listening sessions per year. Maintain and grow Sabathani's existing partner network across health, housing, food security, caregiving, and community engagement organizations. Represent Sabathani at community meetings, coalitions, and events. Confirm next steps in writing after partner conversations to ensure clear, accountable collaboration. Qualifications Education: High school diploma or GED required; associate or bachelor's degree in social work, human services, public health, gerontology, or a related field preferred. Experience: Minimum 2+ years of experience in senior services, community outreach, case management, volunteer coordination, or a related field. Experience working with older adults, particularly African American and African elders, strongly preferred. Experience facilitating groups or community events preferred. Lived experience or deep ties to the South Minneapolis community a plus. Knowledge, Skills and Abilities: Working knowledge of aging services, caregiver supports, and community resources, including public benefit programs such as Medical Assistance and SNAP. Strong relationship-building skills with the ability to establish trust with elders, caregivers, volunteers, and partner agencies. Commitment to culturally responsive, trauma-informed practice and respectful, unhurried engagement with elders. Strong organizational and follow-through skills, with the ability to manage intakes, service plans, volunteer matches, and reporting deadlines simultaneously. Ability to communicate clearly and effectively via oral and written means, including plain-language communication with seniors and written reporting for funders. Ability to exercise sound, independent judgment and maintain appropriate boundaries and confidentiality. Working knowledge of Outlook, Word, and Excel; experience with Salesforce or other data tracking systems a plus. Licenses and/or Certifications: Valid MN Driver's License and access to reliable transportation preferred. Must pass a criminal background check. Working Conditions and Environment: This position is based at Sabathani Community Center, with regular travel to community sites and partner agencies. The building is accessible and smoke-free. This role may occasionally require evening and/or weekend hours to support Caregiver Circles, resource fairs, and community events.
