Position Summary
Working in accordance with the established Everstand Policies and Procedures, federal and state regulations as well as the applicable State Board(s) of Nursing to provide administrative and clinical healthcare services as well as direct nursing services to youth placed within the program. The Caminos Medical Coordinator is an active member of program leadership responsible for care coordination, oversight and administration of medications; coordination of routine and speciality medical and dental care utilizing a network of community providers; health education for youth and staff; and all administrative tasks.

Essential Functions
Administrative Duties
- Model and act in accordance with Everstand's core values.

- Provide oversight and coordination of medical care for the Caminos shelter program, including the delivery of high-quality services in accordance with regulatory timeframes to ensure that youth are provided with safe and supportive programming.
- Ensures all Caminos Program residents receive quality and timely medical and dental care in accordance with state and federal regulations.
- Responsible for developing, maintaining and overseeing working relationships with community providers to ensure all medical appointments and follow-up recommendations are documented, monitored and reported according to state and federal standards.
- Arrange and communicate with staff for all in house and outpatient medical appointments/services to include hospital emergencies and/or admissions, medical appointments, laboratory services, vaccine clinics, x-rays, and ultrasounds.
- Schedule appointments, accompany youth as necessary on specialty medical appointments, and communicate provider's questions/concerns to ensure accuracy and patient safety.
- Produce a daily resident appointment list and ensure all staff are aware via email and hard copies in staff mailboxes.
- Audit, track, and schedule required physical, vision, and dental appointments.
- Response for the initial training and renewal process for non-medical staff to be certified by the State in medication administration, including completion of purchase orders.
- Organize and maintain resident medical charts, physical and electronic, utilizing program required agency and federal databases.
- Update and file patient medical records, including retrieval and copies of medical records necessary for staff reports and outpatient services.
- Assures completeness of documentation for the clients and for associated nursing functions such as the MAR, nursing screening exams, PPD screens, vaccinations, vital signs, laboratory logs, peer reviews and other quality initiatives, nursing sick call screens, consultations and referrals, in service training, and other activities of the healthcare suite.
- Ensures completeness of documentation of medical services provided to clients as required in federal and state databases, including the completion and submission of medical serious incident reports.
- Responsible for timely submission of Treatment Authorization Requests, or other forms required for reporting and billing purposes.
- Inventory and maintenance of all administrative and medical supplies and medications.
- Maintains regular and consistent communication with Caminos Medical Services Administrator and the designated ORR Medical Coordinator, including participation in regular Everstand and ORR sponsored training and guidance updates.
- Performs administrative duties as requested by supervisor, including researching and updating protocols/procedures, obtaining educational materials, coordinating care at outlying facilities, tracking billings and expenditures, organizing in service training, assuring regulatory compliance of the health suite.
- Collaborate with other departments for various service needs, including but not limited to:
- Ensuring comprehensive care coordination.
- Ensuring access to mental and behavioral health services.
- Providing information, psychosocial education and local community resources as needed to identified sponsor placements.
- Work in partnership with the ESS Supervisor to schedule medical transportation
- Demonstrates an understanding of cultural differences and perspectives, including the ability to develop systems and practices that are inclusive of all staff members and responsive to the cultural needs of the client population served.
- Evaluates productivity of the medical department and develops plans to improve efficiency.
- Oversees patient and staff satisfaction through excellent customer service, communication, teamwork, safe practices, attendance, punctuality, and strong leadership.
- Makes sound employment practices by adhering to all Everstand policies, procedures, and standards, local, federal and state regulations and employment laws.
- Demonstrates knowledge of and familiarity with both Everstand and Office of Refugee Resettlement Policy and Procedures relating to the care and supervision of Unaccompanied Children. This includes knowledge of legal background and authority governing the practice as related to working with Unaccompanied Children.
- Demonstrate a willingness and ability to learn and evolve with changes in the organization and in health care.
- Obtain and maintain certification as a "user" in Everstand's electronic health record software system, Cx360.
Clinical Duties
- Carries out physician orders and provides day-to-day nursing care as needed, including administering medications and vaccinations, simple lab tests, phlebotomy, throat culture, support to pediatrician, client education, interpretation of medical information, accompanying clients to surgical or extensive medical/emergency procedures, nursing intake, first aid, and emergency/sick call triage.
- Responsible for preparation of treatment room(s) for patients.
- Assist the providers and/or nurses during exams with drawing labs, changing dressing, removal of sutures, etc.
- Explain treatment procedures to patients and assist with education.
- Instruct patients about medications and special diets.
- Medication administration implementing the six (6) Rights of Medication Administration.
- Medication administration with basic knowledge of medications and their use, drug reactions, and methods of administration.
- Medication administration consisting of: observing the youth for changes in physical conditions and/or behaviors, reporting these observations as soon as possible, receiving the prescribed medication, storing the medication safely, ensuring medications are taken, recording information promptly and correctly on appropriate forms.
- Report medical emergencies immediately to designated personnel.
- Maintain confidentiality of all pertinent health information to assure youths' rights are protected in adherence to HIPAA of Privacy and Security.
- Takes vital signs and/or observes patients to collect information and prepares report or notifies Registered Nurse.
- Keep work area(s) free of hazardous conditions to assure that all established safety rules and regulations are followed at all time in adherence with Infection Control Standards.
- Advocate on behalf of the youths' best interest and collaborate with their treatment team to ensure their health and safety.
- Facilitates training, education and assessment of skills, along with maintaining requirements for medication administration personnel as necessary.
- Complete CDC and OSHA training for hazardous waste, TB management, and blood borne pathogen standards.
- Must be willing to perform tasks that may involve exposure to the patients' blood and/or body fluids.
