About myPlace Health myPlace Health was founded in 2021 by mission-aligned healthcare leaders and organizations that are committed to drastically improving health outcomes, quality and experience for vulnerable older adults and frail seniors. We specialize in providing value-based, comprehensive care and coverage for older adults with significant needs so they can thrive in the homes they love and in the communities they cherish. Our mission is simple: to enable older adults to live the independent lives they deserve. We pursue this mission through our myPlace PACE (Program of All-Inclusive Care for the Elderly) model, which provides seamless primary care, integrated health plan coverage, personalized social engagement, and customized services delivered in the participant’s preferred place. myPlace Health is building a mission-driven team that shares our passion for redefining the way older adults experience care as they “age in place” in the community. This is a unique opportunity to take on one of our country’s most challenging healthcare problems and join a fast-growing, dynamic team as we prepare to scale our mission to serve more markets. About This Role As a Licensed Physical Therapist Assistant (LPTA) in our PACE program, you’ll play a vital role in helping older adults maintain their independence, mobility, and overall well‑being. Working under the guidance of a Physical Therapist and as a valued member of our Interdisciplinary Team (IDT), you’ll bring therapeutic care to life—delivering compassionate, hands‑on support tailored to each participant’s unique goals. This is more than a clinical role—it’s an opportunity to truly connect. You'll build meaningful relationships, support whole‑person care, and make a lasting difference in the lives of frail seniors. Whether you're helping a participant walk more confidently, recover from a hospitalization, or simply stay active and engaged, your care will be essential to their ability to thrive at home and in the community they love. If you find joy in helping others move forward—literally and figuratively—and you're inspired by mission‑driven work in a collaborative, supportive environment, we’d love to welcome you to the team. What Does Success Look Like In This Role? Hands‑On Healing: Deliver one‑on‑one, personalized therapy interventions following care plans created by the Physical Therapist. Mobility Mentorship: Guide participants through therapeutic exercises, balance training, gait support, and physical modalities that improve daily function and quality of life. Attentive Observation: Monitor participants during sessions, track progress, and share valuable insights with the Physical Therapist to support continuous care planning. Respectful, Person‑Centered Care: Create a safe, comforting, and empowering space during each session—adapting your techniques to each individual’s needs. Supportive Education: Teach participants and caregivers how to safely use assistive devices, practice mobility techniques, and follow home exercise programs with confidence. Collaborative Spirit: Work closely with the Physical Therapist and other members of the Interdisciplinary Team to ensure coordinated, holistic care for every participant. Continuity of Care: Support transitions between home, hospital, or skilled nursing settings by helping ensure therapy services stay on track and goals are maintained. Welcoming Environment: Keep therapy spaces clean, organized, and inviting—creating a place where participants feel safe and supported. Reliable Documentation: Accurately record interventions and participant responses in the Electronic Medical Record (EMR) in accordance with PACE, state, and federal requirements. Team Engagement: Actively participate in daily huddles, team meetings, and training sessions to stay informed, aligned, and inspired. Flexible Support: Step in to assist with other participant care or team needs as they arise—bringing a helpful attitude and a willingness to contribute wherever you're most needed. What Does An Ideal Candidate Look Like? Qualified & Ready to Care: Graduate of an accredited PTA program with an active California license (or ability to obtain one). Prepared for the Unexpected: CPR certification (or willingness to obtain within 30 days of hire). Experienced in Geriatric Care: At least 2 years of PTA experience, preferably with older adults or medically complex participants. Rooted in Purpose: A genuine passion for supporting frail older adults through compassionate, person‑centered care. Strong Communicator: Ability to engage participants, caregivers, and teammates with empathy, clarity, and positivity. Adaptable Clinician: Comfortable adjusting interventions based on participant needs, goals, and physical capacity. Confident in Hands‑On Support: Skilled in mobility assistance, transfers, and therapeutic interventions. Detail‑Oriented Documenter: Organized and timely when it comes to recording care in the EMR and managing schedules. Tech‑Savvy: Proficient with electronic medical records and basic computer applications. Reliable Teammate: Punctual, dependable, and always ready to pitch in with a kind and can‑do attitude. Other Pertinent Information Onsite Presence – This is a 100% onsite position supporting our vibrant South Los Angeles market. Your in‑person presence is key to building strong team connections and delivering high‑quality, coordinated care. Consistent Schedule – The regular work schedule is Monday through Friday, from 8:00 AM to 5:00 PM, supporting a stable routine for both staff and participants. The Physical Requirements Helping Hands & Steady Support: Ability to safely lift, push, pull, or assist participants and equipment—up to 50 pounds—using proper body mechanics and assistive tools when needed. On Your Feet & On the Move: Expect frequent walking, standing, bending, reaching, and movement throughout the day as you guide participants through therapy sessions and navigate the clinic. Safe Transfers & Mobility Assistance: Confidence and skill in supporting participants with transfers, walking, and positioning—all while ensuring safety and dignity. Sharp Senses: Visual and hearing ability to monitor participant responses, observe safety cues, and respond quickly when something doesn’t feel right. Hands‑On Tools: Comfortable using therapy equipment, assistive devices, and documenting care using electronic systems. Ready for the Unexpected: Able to respond calmly and quickly in the event of emergencies, whether supporting a fall or assisting during a clinic evacuation. Health & Safety First: Occasional exposure to bodily fluids or health‑related risks—standard precautions and PPE keep everyone safe. The Work Environment Clinic & Day Center: Most of your time will be spent in our on‑site therapy gym, treatment rooms, and participant activity areas. These spaces are thoughtfully designed to be safe, accessible, and uplifting for older adults with a range of physical and cognitive needs. Office‑Based Work: When not delivering hands‑on care, you'll document therapy sessions, collaborate with team members, and participate in care planning in an office setting equipped with EMR systems and communication tools. Home & Community Visits: Occasionally, you’ll support therapy or mobility assessments in participants’ homes or other care settings (such as skilled nursing facilities or hospitals), helping ensure smooth transitions and consistent care. Travel Between Locations: Some travel between program sites or participant homes may be required. Flexibility and reliable transportation are important to support care continuity across settings. Team‑Driven Culture: You’ll be part of a warm, collaborative interdisciplinary team—including nurses, physicians, social workers, activity coordinators, and fellow therapists—working together to provide whole‑person, mission‑driven care. Participant‑Centered: Every day, you’ll build meaningful relationships with older adults and their caregivers, empowering them to regain strength, confidence, and independence. Safety First: We uphold strong standards for infection control, provide appropriate personal protective equipment (PPE), and prioritize a safe and respectful environment for both participants and staff. $70,600 - $84,700 a year What’s In It for You?
