LPN Job Details

• Job Title: Licensed Practical Nurse (LPN)

LPN Job Requirements

• Active New York Licensed Practical Nurse (LPN) licensure
• Current Basic Life Support (BLS) certification through the American Heart Association or RQI
• At least 1 year of LPN experience, preferably in long-term care, skilled nursing, or a comparable post-acute care setting
• High school diploma or GED
• Ability to provide safe, compassionate nursing care to adult and geriatric residents
• Ability to work scheduled night shifts, every other weekend, and required holidays
• Ability to follow infection prevention practices and safely manage potential exposure to bloodborne pathogens

Long-Term Care LPN Responsibilities

• Deliver direct nursing care to long-term care residents in accordance with established care plans, facility policies, and the LPN scope of practice.
• Monitor residents' physical, emotional, and psychosocial conditions and promptly report changes to the appropriate nurse or supervisor.
• Administer medications and treatments as authorized, while documenting care accurately and completing required reporting.
• Support residents with activities of daily living, including hygiene, grooming, dressing, toileting, incontinence care, repositioning, and skin care.
• Promote resident safety by responding to call lights and alarms, supporting fall-prevention measures, and maintaining a clean and secure environment.
• Assist with nutritional needs, including meal support, nourishment delivery, adaptive equipment, and monitoring resident preferences and intake.
• Encourage restorative and rehabilitative activities such as range of motion, ambulation, transfers, proper positioning, and use of assistive devices.
• Respect and advocate for resident rights, preferences, refusals, and concerns while maintaining dignity and confidentiality.
• Collaborate with nursing colleagues and the interdisciplinary care team to support high-quality resident outcomes.
