Occupational Therapist Full-Time

Occupational Therapist (OT)Help residents regain independence through purposeful care. Sierra Valley Rehab Center in Porterville, CA is a skilled nursing and rehabilitation facility committed to delivering high-quality, resident-centered care. We are currently seeking a licensed and compassionate Occupational Therapist (OT) to join our therapy team.Position Summary:As an Occupational Therapist, you will evaluate, plan, and provide therapeutic interventions for residents to improve their ability to perform daily activities. You will work closely with interdisciplinary teams to deliver personalized care plans and support recovery goals in a post-acute care environment.Key Responsibilities:Evaluate patients within 24 hours of physician referral or document cause for delayDevelop and implement individualized treatment plansProvide direct therapy services and document outcomes in accordance with facility, state, and federal requirementsSupervise Occupational Therapy Assistants (COTAs) and aides as per state guidelinesEducate residents, families, and caregivers on therapy goals and home programsParticipate in discharge planning, staff training, and in-service programsAttend care conferences and communicate patient progress to the interdisciplinary teamRecommend adaptive equipment and modifications to support functional independenceMaintain therapy equipment and report any maintenance needsQualifications:Bachelor's, Master's, or Doctorate in Occupational Therapy from an accredited programCurrent California OT license in good standingCPR certification (or obtained upon hire)Experience in skilled nursing or post-acute care preferredProficiency in EMR systems such as Casamba or PointClickCareStrong clinical, organizational, and communication skillsPhysical Requirements:Ability to assist residents in therapeutic movements and mobility tasksFrequent standing, walking, pushing, and liftingAble to lift 50+ lbs and perform physical care duties as neededWhy Sierra Valley Rehab Center?Competitive starting wage of $60/hourFlexible shift availabilitySupportive and collaborative work environmentClean, modern facility with strong clinical leadershipOpportunity to make a meaningful impact in residents' livesApply today.
