The nurse provides home‑based care in the Perinatal, Palliative, and Hospice Programs and adheres to all policies, procedures, and the State Nurse Practice Act. Definitions Essential Care Perinatal Program – provides support, education, and guidance to expectant parents and families whose unborn child has a life‑threatening illness. The program helps families prepare for and cope with physical, emotional, and spiritual challenges at diagnosis and throughout pregnancy. Essential Care Palliative Care Program – a home‑based palliative (symptom‑control) care program that coordinates medical services and delivers psycho‑social and spiritual support to seriously ill children and their families. The RN coordinates physical, emotional, social, and spiritual needs and collaborates with multiple specialties and pediatricians in the community. Essential Care Hospice Program – provides pediatric‑specialized hospice care focused on quality of life, comfort‑oriented medical care, social support, and emotional and spiritual guidance for patients and families in the final stages of a terminal illness. The RN delivers direct care to the patient. Essential Duties and Responsibilities All Programs: Perinatal, Palliative, and Hospice Care Possess, articulate, and demonstrate advanced pediatric specialty knowledge of serious illnesses Demonstrate a bias for advocacy, act on important needs and issues, remain proactive and creative, and maintain professional stability under pressure Establish and maintain positive collaborative relationships with community medical providers, support staff, specialty/facility care providers, and intra‑agency teams Provide patient/family/caregiver teaching and resource information in all situations Organize and prioritize complex assignments Complete assignments and documentation in a timely fashion and according to policy Maintain security and HIPAA confidentiality of all patient medical records Report unusual events and complaints per policy Have schedule flexibility to meet patient needs and accept assignments per program policy for weekend, holiday, and on‑call scheduling Provide education to patients, families, and healthcare or community organizations about perinatal, palliative, and hospice care services, emphasizing understanding of each program’s unique benefits Perinatal RN Responsibilities Provide consultative and case‑management support to mothers facing a potentially fatal or severe life‑limiting illness before delivery Advocate in anticipation of unborn patient, mother, and family needs Essential Care Palliative Care RN Responsibilities Focus on assessment, teaching, and coordinating case‑management referrals Collaborate and advocate with community resources (medical, spiritual, psycho‑social) Essential Care Hospice RN Responsibilities Perform comprehensive assessments to identify physical, emotional, spiritual, environmental, and safety needs related to disease and symptom management to promote patient well‑being, quality of life, comfort, and dignity Demonstrate competency in advanced pediatric clinical services and devices Receive and implement provider orders Prepare and maintain a Plan of Care Participate in and promote an interdisciplinary team approach to all patient care Reconcile and teach medication use and safe delivery Qualifications Education / Certification Graduate of an accredited school of nursing with current RN licensure in the State of New York Certification in Hospice and Palliative Care Nursing preferred Valid driver’s license, free of serious violations, and a vehicle required for use when on duty or on‑call Experience Years of nursing practice as a professional nurse with pediatric experience preferred Salary: $79,950.00 – $101,400.00 per year #J-18808-Ljbffr
