About the Role
As a Licensed Practical Nurse (LPN) at Uniontown Nursing and Rehab, you will provide high-quality patient care to our residents, ensuring their physical, emotional, and spiritual needs are met. You will work collaboratively with our interdisciplinary team to develop and implement individualized care plans, administer medications, and perform various medical procedures.
Responsibilities
• Provide direct patient care to residents, including administering medications, wound care, and vital sign monitoring
• Develop and implement individualized care plans in partnership with the interdisciplinary team
• Perform various medical procedures, such as injections, and monitor patient response
• Maintain accurate and detailed records of patient care and medications administered
• Collaborate with colleagues to ensure seamless communication and care coordination
• Participate in quality improvement initiatives to ensure optimal patient outcomes
