Case Manager
Laboratory to Combat Human Trafficking
Overview Case managers work to facilitate patient care by assessing patient needs, evaluating treatment options, creating treatment plans, coordinating care, and gauging progress. Oftentimes case managers work with physicians, social workers, families, and human services providers. The overall goal for case managers is to improve clinical outcomes, increase patient satisfaction, and promote cost-effectiveness. Case Management Coordinate and facilitate patient care through assessment, evaluation, planning, and implementation Create care plans and comprehensive assessments for specified risk patients related to care management needs and SDOH Ability to plan, implement, and evaluate individual client care plans Communicate patient needs, determined by the assessment, to a patient, families, physicians, and nurses to ensure high quality care Works collaboratively with members of the health care team in the development of the plan of treatment, and follow up accordingly Develops working relationship with leaders in community resource groups, representing the agency at meetings and on various committees that address the social needs of patients Acts as resource person for other members of the health care team, providing in-service education and consultation in understanding the social and emotional factors related to health problems Provide onsite crisis intervention and emotional support as immediate health needs arises, coordinate behavioral and mental health referrals to appropriate specialists Maintains accurate and timely records including clinical and progress notes according to department policy Manage discharge plans upon completion of treatment Coordinate with third party payers on a regular basis Working knowledge of multi system outreach programs related to health care delivery, clinical education, and health related services Knowledge of transportation and other barriers to care that may be encountered by client Medical (Can only be performed by RN) Triage patients over the phone Medication Reconciliation Acts as resource person for other members of the health care team, providing in-service education and consultation in understanding the social and emotional factors related to health problems Other Skills Comfortable working independently and collaboratively Possesses refined problem-solving skills Effective and clear communication skills (i.e., written and verbal) Interviewing skills Strong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies in a diverse community Knowledge of community agencies and resources Ability to communicate medical information to health care professionals and care coordinators over the telephone Skill in use of personal computers and related software applications, including e-mail Skill in organizing resources and establishing priorities Creative and analytical thinking Performs miscellaneous job-related duties as assigned by supervisor Requirements Bachelor's degree in nursing and are a registered nurse in the state of practice or Master of Social Work Degree from an accredited program required Licensed Clinical Social Worker (LCSW) or Licensed Clinical Social Worker Associate level (LCSWA) in good standing with the NCSWLB Case management experience required Seniority level Entry level Employment type Full-time Job function Health Care Provider Industries Non-profit Organizations #J-18808-Ljbffr
