If so, we have an immediate opportunity for you to join our team at Valor Hospice.Job Summary:The registered nurse plans, organizes, and directs hospice care with an emphasis on community health education and experience. As an RN Hospice, you will utilize your nursing skills to provide professional care and support to patients and their families within their homes and communities.Duties & Responsibilities:Patient Care:Complete comprehensive assessments of patients and families to determine hospice needsDevelop and implement care plans based on nursing diagnosis and palliative nursing actionsAdminister medications and treatments as prescribed by physiciansAssist patients with activities of daily living and promote self-sufficiency and comfort careCoordinate patient care as a Case ManagerCommunication:Maintain accurate clinical notes and recordsCommunicate with physicians and community health-related persons to coordinate careTeach patients and families self-care techniques and provide instructionsProvide a safe environment for patientsWork with interdisciplinary group members to meet emotional needs of patients and familiesAdditional Duties:Participate in on-call duties as requiredEnsure availability of necessary equipment and servicesSupervise ancillary personnel and delegate responsibilities when requiredMaintain and upgrade professional knowledge and skills through continuing educationActively participate in quality assessment performance improvement activitiesJob Requirements:Graduate of National League for Nursing accredited school of nursingMinimum of two years of nursing experience, preferably in public health, home care, or hospiceCurrent licensure to practice professional nursing in the stateExcellent observation, problem-solving, and communication skillsAbility to organize and prioritize workload independentlyCPR certificationValid driver's license and insured automobile in good working orderSelf-directed and able to work with minimal supervisionLocation: Sierra Vista and the Surrounding AreaJoin our team at Valor Hospice and make a difference in the lives of patients and families in need of hospice care.
