Responsibilities
- Provide direct patient care for long-term care residents, including routine assessments and daily living activities.
- Administer medications and treatments per plan of care and facility policy.
- Monitor residents’ conditions, document changes, and communicate with the nursing team and supervising RN.
- Maintain patient safety, infection control, and compliance with privacy and regulatory requirements.
- Collaborate with the interdisciplinary team to implement and adjust care plans.
- Complete accurate documentation in electronic health records and ensure continuity of care.

Requirements
- Active LPN license in good standing
- Ability to work night shift (18:00–06:30) with weekend rotations
- Willingness to commit to a 17-week travel assignment
- Ability to provide patient-centered nursing care in a skilled nursing/long-term care setting
