Overview Matrix Medical Network is seeking a compassionate Nurse Practitioner to provide in-home health assessments and support quality patient care. In this role, you will conduct comprehensive evaluations, coordinate care plans, and collaborate with multidisciplinary teams to improve outcomes and reduce healthcare costs. You’ll leverage your clinical expertise to support health management initiatives for insurance plans and government agencies. Responsibilities Conduct in-home health assessments and comprehensive evaluations Coordinate care plans and care management activities Collaborate with multidisciplinary teams to improve patient outcomes Support health management initiatives for insurance plans and government programs Promote cost-effective, high-quality care Qualifications Nurse Practitioner license (ARNP/NP) Experience in in-home or community-based care is preferred Strong clinical skills and ability to coordinate patient care across teams #J-18808-Ljbffr
