Key Responsibilities

  • Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in alignment with patient needs, goals, and physician direction
  • Maintain accurate, timely medical record documentation including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and payer guidelines
  • Delegate and direct discipline-specific therapy services for assigned patients, ensuring functional treatment programs support effective return to community activities
  • Serve as an educator for patients, families, students, and coworkers; participate in student programs by providing supervision, developing learning objectives, and completing student evaluations
  • Adhere to discipline-specific State Practice Act and all applicable regulatory requirements; maintain professional licensure in good standing and fulfill all state-mandated continuing education requirements
  • Actively contribute to organizational performance improvement initiatives and apply professional development learnings to meet strategic objectives

Required Qualifications

  • Active Physical Therapist licensure in good standing in the state of Minnesota
  • Prior clinical experience as a Physical Therapist required — new graduates will not be considered
  • Proficiency in clinical reasoning, treatment planning, and goal-setting focused on patient progression toward discharge
  • Knowledge of and adherence to state and federal documentation and compliance standards
  • Commitment to patient and employee safety practices, including safe patient handling and infection control protocols

Preferred Experience / Skills

  • Experience in an outpatient physical therapy setting
  • Familiarity with functional quality indicator assessments and letters of medical necessity
  • Active involvement in continuing education and professional organizations within the physical therapy field
