Job Summary We are seeking a compassionate Medical Social Worker to support home health patients and their families by addressing psychosocial, emotional, and resource needs. Responsibilities Assess patient psychosocial and environmental needs Assist with discharge planning and long-term care arrangements Provide counseling and emotional support to patients and families Connect patients with community resources and financial assistance programs Complete required documentation per Medicare/home health standards Participate in care coordination with interdisciplinary team Qualifications Master’s degree in Social Work (MSW) Active Florida license (LMSW/LCSW preferred) Home health or healthcare experience preferred Strong knowledge of community resources Excellent communication and organizational skills Benefits Flexible schedule Mileage reimbursement This role requires a Level 2 background screening in compliance with Florida AHCA regulations prior to employment. #J-18808-Ljbffr Palms Home Care
