Key Highlights Weekly pay: $2,299–$2,395 per week Guaranteed hours: 40 per week Contract duration: 13 weeks Shift: Daytime, 40 hours Start date: 08/24/2026 Setting: Home health care – hospice Responsibilities Provide direct physical therapy services to home health patients, including assessment, treatment planning, and ongoing rehabilitation Develop, implement, and adjust the patient’s plan of care in collaboration with the interdisciplinary team Utilize therapeutic modalities and rehabilitation equipment; apply splints, orthoses, prostheses, wheelchairs, and ADL equipment as appropriate Document assessments, interventions, and outcomes; ensure compliance with home health documentation standards and regulatory requirements Educate patients and families on safety, exercise programs, activities of daily living, and caregiver techniques Coordinate care with other providers; participate in care conferences and contribute to the overall patient care plan Travel to patient homes and maintain appropriate equipment; manage time effectively in a home-based setting Requirements Massachusetts Physical Therapist license (active) Home health experience with proficiency in OASIS (certified home health experience) 2+ years of combined acute hospital care and home health experience (preferred) Valid driver’s license and access to reliable personal vehicle Active American Heart Association BLS certification Ability to work independently with minimal supervision in a home health setting Benefits and Support Guaranteed hours 24/7 recruiter and travel support Dedicated support throughout the assignment Nationwide assignments Extension opportunities Fast credentialing
