Hospice Admissions RN (PRN)

Location: HMC Administration Office 3231 SW 34th Ave Ocala, FL 34474, USA Join a compassionate, mission-driven team with Hospice of Marion County , part of Empath Heal th. We’re looking for a dedicated and empathetic Admissions RN to make a meaningful impact by guiding patients and families through the admissions process—providing care and support in homes, healthcare facilities, and hospitals throughout Marion County. Schedule: PRN, minimum of 64 hours a month availability. Hospice experience required. What you'll Do The Admissions RN is a first responder who assesses eligibility for Empath Health Network of Care Advocating for optimal access to the right care at the right time in any care setting. The Admissions RN completes the admission process based on care program specifics, guidelines, and patient-specific needs. Coordinate and assist with the Professional Liaison to deliver educational, in-service and outreach tools in the hospital. Works directly with community referral sources to educate regarding Empath Health programs and services. Complies with RN On Call scheduling needs as well as vendor credentialing process. What You'll Need Hospice experience strongly preferred Education and/or Experience: Associate’s degree (A.A.) or equivalent from two-year college or technical school; or six months to one year related experience and/or training; or equivalent combination of education and experience. Licensure: Valid Florida license as a Registered Nurse Valid Florida State Driver’s License For field-based positions, employees must have reliable transportation which will enable them to perform tasks and responsibilities in a timely and appropriate fashion. Must provide proof of valid automobile insurance, a copy of which will be placed in the employee’s HR file.
