Continuum Care Services Inc. is seeking a empathetic, compassionate,positive and personable individual to join our Community Support Team (CST) asthe Qualified Professional (QP). The (QP) is to provide Community Support Teamservices that will include direct and indirect interventions. The developmentof the Person-Centered Plan is the responsibility of the QP on the team. CST isintended for individuals with mental illness, substance abuse, disorders, orboth who have complex and extensive treatment needs. As a service provider, theQualified Professional must assist the consumer to develop critical dailyliving and coping skills, gain access to necessary services, and reducepsychiatric and addiction symptoms. This includes, but is not limited to:assessing client needs, arranging services, implementing comprehensive serviceplans, working in conjunction with Community Support Team Leader (LP),paraprofessional and associate professional staff, advocating on behalf of theclient, completing required documentation, and other duties as assigned.
Job Type: Full-time or Part Time Hours: Monday - Friday 9am-5pm

Pay : $18.00 per hour

Education :

• Bachelor's degree (in Human Servicesfield) & 2+ years full-time experience with population served

• Bachelor's degree (Not Human Servicesfield) & 4+ years full-time experience with population served

• Master's Degree or Higher & 1+ yearsfull-time experience with the population served.

Experience:

• Develops the initial Person Centered Planand its ongoing revisions, and ensures the implementation of the PersonCentered Plan (PCP)

• Assists the Team Leader with behavioral andsubstance abuse treatment interventions

• Assists with the development of relapseprevention and disease management strategies linkage to the most clinicallyappropriate and effective services including arranging for psychological andpsychiatric services

• Assists with crisis interventions

• Consults with identified medical (forexample, primary care and psychiatric) and non-medical providers, engagescommunity and natural supports, and includes their input in the person-centeredplanning

Occupational Certification/Licenses

• North Carolina Driver's License
