Summary

The Home Health & Hospice Registered Nurse makes home visits and develops a nursing plan of care and implements that nursing plan of care to patients at their place of residence in accordance with the philosophy, goals, and objectives of Cozad Community Health System.

Essential Duties and Responsibilities

Serves as the primary nurse/case manager for all assigned patients. Develops the plan of care for each patient, in coordination with the primary care provider, and oversees that plan of care for effectiveness.

Assesses patient’s condition at each home visit. Documents that assessment accurately and reports to the physician as needed.

Provides direct patient care and demonstrates knowledge in home care/hospice as well as scientific and technical principles, drugs, and therapeutics. Carries out physician orders, dressing changes, venipuncture, home infusion, medication reconciliation, etc. as specified in the plan of care.

Performs and maintains documentation.

Follows all HIPAA guidelines and promotes and ensures patient safety.

Fulfill mission statement by performing job duties with a high level of customer service while contributing to a positive and professional environment.

Special projects and other duties as assigned.

Supervisory Responsibilities

No supervisory responsibilities.

Education and/or Experience

Graduate of an accredited school of nursing. Previous Home Health & Hospice experience preferred. Electronic Health Records experience and management experience preferred.

Licensure/Certification

Current Registered Nurse licensure in the State of Nebraska.

Basic Life Support (CPR)

Advanced Cardiac Life Support (ACLS) preferred

Valid Driver’s License

The employee is occasionally required to stand; walk; reach with hands and arms and stoop, kneel, crouch, or crawl. The employee must frequently lift and/or move up to 25 pounds; must occasionally push, pull and assist with lifting up to 300 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus.

Ability to operate a motor vehicle and complete errands based on business needs.

Work Environment

While performing the duties of this job, the employee regularly works in a medical clinic setting and patient residences. The noise level in the work environment is usually moderate.
