PRN RN Location: The Ranch at Dove Tree, Lubbock, Texas About the Job PURPOSE STATEMENT: The Registered Nurse is responsible for providing nursing care to clients, supervising care treatment and services of client care provided by LPNs, CNAs, and other staff on assigned unit and shift. The RN develops, implements, and evaluates an individual plan of care for each client, performs assessment and re‑assessment, communicates information with the interdisciplinary team, and reports client concerns and changes in condition to supervisor and/or provider promptly. Roles and Responsibilities Provides and coordinates care by assessing physical and behavioral health needs of client, develops and implements nursing care plans, maintains medical records, and educates clients and their families while ensuring client safety at all times. Assesses clients’ status on an ongoing basis and communicates findings to providers in a timely manner. Ensures nursing treatment plans are specific to assessed needs and that the initial treatment plan is completed upon admission. Obtains provider’s orders accurately, documents appropriately, and verifies that all required elements are present. Accurately administers medications per orders, utilizing at least two client identifiers. Notifies providers of significant changes in status such as difficulties with detox, medical emergencies, pertinent lab results, or changes in medical status regarding response to medications. Assesses and documents effects of PRN medications. Maintains accountability of medication administration, recording, control, and security, including narcotics. Completes admission and discharge processes efficiently and accurately as directed. Confers regularly with treatment team (e.g., nurses, therapists, program managers, provider) regarding client care and attends unit activities such as therapy groups, family meetings, staffing and unit meetings. Provides education and explanation to clients, families, and co‑workers regarding medication usage, common side effects, client rights, and other topics necessary for optimal client care. Monitors client acuity, census, staffing needs, and escalates coverage needs to supervisor/Charge RN. Ensures thorough, accurate documentation for all clients on each shift; client assignment sheets are completed daily and tasks are carried out as assigned. Communicates thorough hand‑off reports to incoming staff and supervisors. Education/Experience/Skill Requirements Graduation from an approved/accredited school of nursing. One or more years’ nursing experience in a clinical area of service provided by the facility (preferred). Experience in a behavioral health/psychiatric/addiction recovery environment (preferred). Knowledge of and experience with psychotropic medications and medication administration procedures. Ability to apply nursing standards and practice within an acute psychiatric setting. Licenses/Designations/Certifications Valid current RN license from the state (or a compact state, where applicable). Certified in Psychiatric Nursing (preferred). CPR, de‑escalation, and restraint certification required (training available upon hire and offered by the facility). First aid may be required based on state or facility requirements. Work Location This position is onsite at the facility and is not a remote position. Supervisory Requirements This position is an Individual Contributor.
