Occupational Therapist (OT) – Home Health | PRN | Flexible Schedule

Anova Care is seeking compassionate and skilled Occupational Therapists (OTs) to join our growing home health team . If you are passionate about helping patients regain independence and improve their quality of life, we would love to hear from you.

In this role, you will provide one-on-one occupational therapy services in the comfort of patients’ homes , helping them safely perform daily activities and maintain independence. This is an excellent opportunity for therapists who enjoy flexibility, autonomy, and meaningful patient relationships .

Key Responsibilities

• Conduct comprehensive patient evaluations and develop individualized treatment plans.

• Provide skilled occupational therapy interventions in the home setting.

• Educate patients and families on therapeutic techniques and safety.

• Monitor patient progress and document outcomes in the EMR.

• Communicate effectively with patients, caregivers, and the interdisciplinary care team.

• Ensure compliance with home health regulations, physician orders, and documentation standards.

• Participate in care coordination and quality improvement initiatives.

• Collaborate with clinical and administrative teams to ensure excellent patient care.

• Perform additional duties as assigned to support patient outcomes.

Qualifications

• Master’s or Doctorate Degree in Occupational Therapy

• Active state licensure as an Occupational Therapist

• 1–3 years of clinical experience in home health, outpatient, or rehabilitation preferred

• Strong patient assessment and treatment planning skills

• Knowledge of home health regulations and documentation requirements

• Experience with EMR systems and Microsoft Office

• Excellent communication and interpersonal skills

• Ability to work independently and manage a patient caseload

Schedule

PRN | Pay-Per-Visit
Flexible scheduling with availability during business hours and occasional after-hours/weekend coverage.

Pay

$70 – $130 per visit
