Full-Time Mentor - Adult Day Services - M-F Days

As a Life Skills Coach you empower individuals with disabilities to gain social and daily living skills, fostering active membership in their communities. Position Summary Life Skills Coach 1 – Full‑time, Monday‑Friday. Schedule:M,W – F8:30 am – 2:45 pm; T8:30 – 3:30 pm (staff meeting). Hourly rate: $20.00 per hour. CPR/First Aid certification provided. Eligible for medical, dental, vision, 401k, tuition reimbursement up to $5,250 per year, 15 days PTO, 11 paid holidays, career advancement pathway, work/life balance, no driving required. Responsibilities Assist individuals with assessments, planning, and evaluation processes. Support individuals in preparing for and attending meetings with the support team and advocate for changes to Service Plans as necessary. Assist in the development and implementation of Individual Service Plans (ISP) consistent with personal goals. Maintain required documentation and data, including ISP, quarterly and annual evaluations, and case notes. Select person‑centered activities and life‑skills learning experiences in accordance with individual desires, ISP, and Individual Program Plan (IPP) objectives as needed. Work with other direct support associates in planning and delivery of activities consistent with individual and service objectives. Supervise individuals while at the service and provide personal care support as required, including nutrition, personal hygiene, and assisting individuals to look their best. Support individuals in finding preferred communication methods, educate on use of appropriate tools, use all technology, alternative communication, and adaptive equipment; train in assistive technology as directed. Follow procedures, standards, guidelines, and best practices established in designated services; comply with regulations and safety requirements. Attend in‑service trainings and meetings as required; perform other duties as assigned. Requirements High school diploma or equivalent. Valid CPR and First Aid certificates (current). 2–4 years of related experience preferred; combination of education, experience, and training acceptable. Knowledge of person‑centered programming approach and ability to create purposeful activities. Active listening, patience, empathy, creative, flexible, problem‑solving, organization, and timely completion of tasks. Positive judgment, discretion, confidentiality handling, policy and procedure interpretation. Basic computer proficiency and ability to input information in service line applications. Personal care assistance, transfer, conflict resolution, calm, collective, ability to lift up to 50 pounds, walk, stoop, stand, kneel, push wheelchairs, tolerate outdoor conditions, rearrange furniture, local travel, criminal background clearance, vaccinations, medical clearance, drug testing.
