Candidates must be located in Tampa, Florida. This role requires 50‑75% travel for face‑to‑face visits with members in Hillsborough County or surrounding counties. Schedule is Monday‑Friday, 8:00am‑5:00pm, standard business hours. We are seeking a self‑motivated, energetic, detail‑oriented, highly organized, tech‑savvy Case Management Coordinator to join our Case Management team. The Case Management Coordinator is responsible for telephonically and/or face‑to‑face assessing, planning, implementing, and coordinating all case management activities with members to evaluate the medical needs of the member to facilitate the member’s overall wellness. The Coordinator will effectively manage a caseload that includes supportive and medically complex members, develop a proactive course of action to address issues presented to enhance short and long‑term outcomes, and identify opportunities to enhance a member’s overall wellness through integration. The Coordinator will determine appropriate services and supports due to the member’s health needs, including but not limited to: Prior Authorizations, Coordination with PCP and skilled providers, Condition management information, Medication review, and Community resources and supports. Required Qualifications Bi‑lingual (fluent in English and Spanish) Must reside in zip code 33617 Willing and able to travel 50‑75% of the time to meet members face‑to‑face in Hillsborough County and surrounding areas in Florida 1+ years of experience in behavioral health or a long‑term care setting Computer proficiency in Microsoft Word, Excel, and Outlook Preferred Qualifications Strong preference for previous Field Case management and discharge planning experience Ability to multitask, prioritize and effectively adapt to a fast‑paced changing environment Effective communication skills, both verbal and written Managed care experience Education Bachelor’s Degree required, preferably in Social Service or a related field (social services, psychology, health administration) Anticipated Weekly Hours 40 Time Type Full time Pay Range The Typical Pay Range For This Role Is $21.10 - $40.90 per hour. This range represents the base hourly rate or base annual full‑time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short‑term incentive program in addition to the base pay range listed above.
