Family Nurse Practitioner

Family Nurse PractitionerThe Family Nurse Practitioner provides comprehensive primary care services to patients across the lifespan, including pediatric, adolescent, adult, and geriatric populations. The practitioner is responsible for health assessment, diagnosis, treatment, health promotion, disease prevention, chronic disease management, patient and family education, and coordination of care across the continuum. Care is delivered within the practitioner's education, certification, licensure, privilege, and scope of practice, and in accordance with applicable laws, regulations, organizational policies, and any required collaborative practice agreement. The Family Nurse Practitioner works collaboratively within an interdisciplinary care team to improve access, quality outcomes, continuity of care, and patient satisfaction.Essential Functions:Provides primary care services across the lifespan, including pediatric, adolescent, adult, and geriatric carePerforms age-appropriate health assessments, physical exams, and diagnostic evaluationsDiagnoses and manages acute and chronic conditions across all age groupsOrders and interprets laboratory tests, screenings, and diagnostic studiesPrescribes medications and provides age-appropriate patient and family educationPerforms medication reconciliation and monitors treatment effectivenessProvides preventative care, wellness visits, immunization support, and health screeningsDelivers anticipatory guidance based on developmental stage and patient needsEngages parents/guardians in care as appropriate and compliant with regulationsCoordinates care with internal and external providers, schools, and community partners as appropriateIdentifies behavioral health, social, and environmental factors impacting healthCompletes accurate, timely documentation in the electronic health recordSupports appropriate coding, billing, and compliance requirementsMonitors and follows up on test results and clinical communicationsParticipates in quality improvement and patient safety initiativesProvides consultation and support to clinical staff as appropriateMaintains required licensure, certifications, and continuing educationAdheres to all HIPAA, infection control, and regulatory standardsPromotes teamwork, professionalism, and service excellencePerforms other duties as assignedWork-Related Experience:Minimum two (2) years of experience in family practice or primary care preferredExperience working with pediatric through geriatric populationsExperience with preventative care, immunizations, and chronic disease managementFamiliarity with family-centered care and care coordination preferredPhysical Demands and Work Environment:Mental:Strong critical thinking and clinical decision-making skillsAbility to manage multiple patients and competing prioritiesAbility to remain calm and effective in high-pressure or urgent situationsStrong problem solving, analytical, and multitasking abilitiesAbility to deliver compassionate care while maintaining professional boundariesPhysical:Frequent standing, walking, and occasional lifting of patients or equipmentAbility to perform repetitive motions, including typing for EHR documentation.Dexterity to perform procedures (e.g., injections, point of care testing)Exposure to bodily fluids, infectious diseases, and hazardous materialsProlonged sitting at a desk with frequent computer use.Working Conditions:Clinical environment with exposure to infectious diseases, hazardous materials, and emotionally challenging patient cases.Education, Certification, and Licensure:Education: Master's or Doctoral degree in NursingCertification: National certification as a Nurse PractitionerLicensure: Current prescriptive authority and DEA registrationSkills:Strong patient assessment and clinical decision-making skillsAbility to build rapport and establish trust with patientsKnowledge of evidence based primary care practicesStrong documentation and organizational skillsEffective communication with patients and interdisciplinary teamsCommitment to quality, safety, and continuous improvementCultural competence and ability to work with diverse populations
