Schedule: 8-hour shift

Assignment Details:

• Contract Length: 14 weeks
• Guaranteed Hours: 40

Requirements:

• Current State License as a Physical Therapist
• Proof of current CPR certification
• Current driver's license. Must have reliable transportation and insurance
• 1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting
• Previous home care experience - Preferred

Roles & Responsibilities:

• Conduct clinical assessments to identify patient needs and determine appropriateness for care.
• Document and create comprehensive Plans of Care, progress notes, and clinical records.
• Develop and implement treatment programs for neuromuscular, neurological, and orthopedic disorders.
• Instruct patients and families on transfer techniques and the use of therapeutic equipment safely.
• Educate patients/families on therapeutic procedures that respect their age, culture, and learning readiness.
• Collaborate with interdisciplinary teams to ensure patient-centered care.
