Description: The RN implements professional nursing practice centered on the patient as a whole person. The focus is on creating a caring environment centered on healing and wellness. Nursing practices are based on evidence and current research, as well as hospital and department policies and procedures. Regulatory standards and professional guidelines also inform nursing practice. Clinical decision-making incorporates all four phases of the nursing process, critical thinking, and creativity. Nursing excellence is promoted through helping relationships among nursing staff and across other disciplines. Direct patient care activities on the unit include: provision of nursing rounds, assessments, and treatments. Facilitating admission, transfer, and discharge activities is also a key responsibility. Provision of patient and family-centered education, care coordination, and individualized treatment planning are essential components of care. Assessment of patient needs involves performing and documenting a comprehensive assessment in accordance with RN scope of practice. The RN considers the patient’s culture, age, and developmental level during assessments. Identifying patient risk factors and presenting complaints is crucial. Health information may be obtained from family or others when necessary. Objective and subjective data are collected to inform care. The initial admission assessment must be verified if obtained by other team members. A problem list is developed, prioritizing patient needs based on input from the inter-professional team. The RN designs actions and interventions relevant to the patient’s culture and age. Desired outcomes are determined in coordination with the patient and family, in accordance with the medical plan of care. Patient care therapies and procedures are fully implemented with empowerment and clinical autonomy within scope of practice. Procedures, treatments, and processes are explained to patients and families. Actions are initiated to alleviate patient stress and discomfort. Changes in patient condition are recognized, responded to, and communicated appropriately. Ongoing assessment and responses to procedures and therapies are conducted and documented. The RN delegates tasks appropriately and oversees team members in providing patient care. Accuracy and completeness of delegated patient care are monitored. Educational needs of patients related to prioritized problems are identified and addressed. Facilitating the patient discharge process includes executing discharge orders in a timely manner. Potential discharges are identified, and discharge teaching is performed. Coordination of the discharge plan with Case Management and others is ensured. Input into patient/nurse assignments is provided, considering staffing and patient acuity. Active participation in unit-based activities supports organizational values and priorities. A positive group dynamic and healing environment are promoted through words and actions. Nursing excellence is assured in accordance with accepted standards and evidence-based practice. Active participation in department and unit quality improvement activities is expected. A holistic perspective regarding patient care is maintained throughout all activities.
