divh2Job Title/h2pOur Home Care RNs/ppCase manage, assess, plan, develop and implement a comprehensive home health care plan for their patients./ppMeasure vital signs, Administer IVs, change dressings, clean wounds as needed/ppProvides skilled nursing care, preventative rehabilitative procedures, and prescribed treatments with a variety of patient populations within various potentially complex home situations./ppWork closely with a supportive patient care team including physicians, therapy and social services, pastoral care, home health aides and others for your assigned caseload./ppProvide patient and family education for disease management and health promotion as directed by physicians plan of treatment./ppProvide care to the most upmost standard/ppMinimum Qualifications:/pulliNew York State RN license/liliMinimum of one year of recent hands on experience as a Registered Nurse./liliBasic Life Support Certification/liliValid New York State Drivers License and reliable transportation/li/ulh2Posted Salary Range/h2pUSD $100,000.00 - USD $130,000.00 /Yr./ppThis range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidates qualifications, skills, competencies and experience.
