RN Case Manager

Providence Care is hiring full time Registered Nurses to join our growing team by providing Case Management services to patients in their homes, assisted living and long term care facilities who are enrolled in our hospice program.Do you have a passion for helping people? At Providence Care, we provide end of life care to patients and provide support to family members.Responsibilities:Looking for someone with a Hospice heart and passion for providing end of life careProvide Case Management to patients and families, this includes symptom management, medication management, and collaboration with members of the IDGResponsible for overseeing patient plan of care with the collaborating physicianProvide supervisory visits and oversight for LPN and CNAsWill conduct start of care visits and collaborate with Care NavigatorsProvide education to patients, families, and facility staffProvide oversight to Home Health Aides and LPNsQualifications:Must have a RN license unincumbered in the state you are applyingReliable transportation and valid drivers license, clean MVRExperience in Hospice/Home Health is preferredProvidence Care is an Equal Opportunity Employer.
