What You’ll Do As a Case Manager, you’ll be a trusted partner and advocate for consumers in our Home Care program. Your work will include: Conductingthoughtful, person-centered assessmentsto understand each consumer’s strengths, needs, and goals Developingindividualized care plansthat support independence and quality of life Coordinating services and community resources to ensuresmooth, responsive care Monitoring progress and adjusting plans to support the best possible outcomes Advocatingfor consumer rights, needs, and preferences Collaborating closely with asupportive, mission-driven teamand community partners Who You Are You’re empathetic, organized, and driven by purpose.
