Schedule: Monday - Friday, 8:00 AM - 4:30 PM
Compensation: $50-$70/hour (based on experience)
Bonus: $5,000 Sign-On Bonus

Position Overview

We are seeking a dedicated Physical Therapist to join our Sacramento center as soon as possible. This is an urgent opening with a strong preference for direct hire candidates; however, contract and contract-to-hire options will also be considered.

In this role, you will provide comprehensive physical therapy evaluations and treatment for participants aged 55 and older, supporting acute, restorative, and maintenance care needs. You will play a key role in helping individuals maintain independence and improve quality of life through personalized rehabilitation plans.

Interview Process

• Initial 30-minute virtual interview (Microsoft Teams)
• Potential onsite meeting for direct hire or contract-to-hire candidates

Key Responsibilities

• Conduct evaluations and develop individualized treatment plans based on physician orders
• Integrate therapy plans into each participant's Plan of Care (POC)
• Provide hands-on treatment including therapeutic exercise, modalities, and mobility training
• Educate participants and caregivers on exercises, safety, and equipment use
• Collaborate with interdisciplinary team members to coordinate care
• Communicate participant progress, needs, and updates with staff, caregivers, and families
• Make referrals to Occupational Therapy, Speech Therapy, or medical providers as needed
• Perform home visits and document all care and participant responses
• Delegate appropriate therapy tasks to Physical Therapist Assistants and Rehab Aides
• Order, manage, and maintain Durable Medical Equipment (DME), including tracking loans and repairs
• Participate in team meetings, peer reviews, and quality improvement initiatives
• Provide training and guidance to staff to support participant care and safety
• Ensure all documentation is completed accurately and in a timely manner

Qualifications

Required:

• Bachelor's Degree in Physical Therapy
• Graduation from an accredited physical therapy program
• Active Physical Therapist license in the state of California
• Minimum of 1 year experience working with frail or elderly populations

Preferred:

• 2+ years of experience as a practicing Physical Therapist
• Master's (MSPT) or Doctorate (DPT) in Physical Therapy
• Bilingual skills are a plus
